It brings significant changes to the whole end to end process of adult care from assessment through to delivery. It puts people in control of their social care support through self assessment, individual budgets, choice of care and provider, the broker role working on behalf of the individual and further highlighting the safeguarding role of the authority. Running alongside this change is the introduction of the Common Assessment Framework, which builds on the Single Assessment Framework and person centred planning. This includes the requirement to share assessment data across organisational boundaries to provide a holistic view of the individual's care record across the health and social care community. Sheffield commissions care through independent sector providers and has some in house providers of care services. Self Directed Support includes the requirement to provide details to the individual about the actual care provided and therefore a key component included in this work is the ability to monitor care delivery real time and to share this information with the individual electronically. We have a strong and effective working relationship with our local NHS partners and need to ensure that this work builds in our holistic approach across the health and social care community. We are looking for someone who can work with us to develop our approach to this change through modelling the components required across the whole of Adult Care Services activity initially at a conceptual level. We are not starting form a blank page we have completed some preparatory work and are keen to build on this. Person Specification The Successful candidate will be able to demonstrate hands-on knowledge in process and organisational change and a high level of awareness of activities and issues associated with business change, particularly IT enabled change. The individual must demonstrate significant analytical skills, and must be capable of working in a business transformation programme with multiple work streams and touch points between streams. The Candidate must also demonstrate significant expertise with key BA disciplines (e. g. process design, process modelling, process reengineering and optimisation, operations management, organisational design, service delivery business requirements and analysis. )The candidate will be TOGAF certified and have extensive Local Government Experience ideally including Adult Social Care. Any previous exposure to the MEGA EA Modelling tool will be a distinct advantage. This is a full time post 37 hours per week. Daily rate vary from PAYE or LTD Rate of pate..

Duties ancillary to instruction, such as invigilating, correcting, progress tests and other tasks appropriate to the grade (ie Assessing and marking course work and progress tests in both theoretical and practical aspects).  Visit trainees when on work placements for assessment and mentoring.  Be prepared to work at other Mid Kent College locations as required.
